Below is the application for the Town of Lebanon COVID-19 Small Business Relief Grant Fund. Grants of $5,000 are available to qualifying small businesses located in the Town of Lebanon. Please refer to the Eligibility Information section below to see grant qualifications. Businesses that qualify will need to either print the application or stop by the Town Hall to pick up a copy. Completed applications along with 2019 Federal & State Tax Forms (2018 if 2019 is not completed), as well as W-9 Forms must be returned to the Town Hall or sent by email to Diane Nunley at dnunley@lebanonva.net.
